Moskaluk, Jason "Blaize"
Jason passed away comfortably at CKHA-Chatham Campus surrounded by his close friends and family on Sunday, August 27, 2017 at 6:22 pm. at the age of 46.
He was born in Toronto to George and Pam Moskaluk.
Jason spent his life helping others as a paramedic. He was a musician and guitarist who played in many bands and toured across the country.
Loving partner to Kim Bitz.
Proud father of Lucas and Ivy Moskaluk.
Big-Little brother of Lisa Moskaluk.
Sadly missed by many family and friends.
Jason is leaving a big hole in the world, as well as in the hearts of those that had the pleasure of knowing him.
Cremation has taken place.
Family to receive friends at the McKinlay Reception Centre, 463 St. Clair Street, Chatham, ON, for a Celebration of Jason's Life on Friday, September 1, 2017 from 2-5 p.m.
